Below are a series of questions about dietary preferences and accommodations, please do not respond to this email with your answers. Instead answer the following in this Google Form by Friday, March 9th. Let us know if you have any difficulties with the form.
On both days of the forum we will provide breakfast and lunch. Breakfast will be continental style. For lunch we will be providing box lunches, you will see your choices on the Google Form. Please choose one for Wednesday and one for Thursday.
There will be a dinner on Wednesday night, social not working, and completely optional. Please let us know if you would like to attend. The dinner will be at Din Tai Fung, the University District location. It is about a mile walk from the location of the forum, here’s a map if you choose to walk.
We may be taking some photographs during the forum. These photos may be used in communications and reports about the forum to IMLS, and therefore may appear in documents on the forum website. If you would prefer you photo not appear in any of these venues, please let us know by the end of the forum.
Additionally, let us know if there are any other accommodations we can make for you.
